As an “Adopter” you will pay a donation sum. This donation will be applied to prepare more animals for adoption, and is therefore not refundable The undersigned Adopter agrees to be bound by the following terms and conditions: Please initial space provided by each paragraph.

Adopter will provide, all at his or her own expense, proper and sufficient food, water, shelter, kind treatment and proper medical care for the Dog/Puppy at all times. With this contract, you are agreeing to provide monthly care, yearly vaccines, physical and veterinary care as needed.

Having a Dog/Puppy may require preparation such as putting up a cattery, if adopter wants the Dog/Puppy to safely spend time outside. In addition to the cattery, for the safety of the Dog/Puppy , you should put away things that your new Dog/Puppy may want to chew (i.e. plastic bags, wires, chargers, hair ties etc). Adopter agrees to comply with all city, state, and federal laws relating to animals.

If Adopter fails to comply with the terms of adoption at any time during the life of the Dog/Puppy, the Dog/Puppy must be returned to our agency. The Dog/Puppy may not be given away or sold without the written approval of our agency, nor may it be surrendered to a shelter or pound. If it is decided that the Dog/Puppy is to be returned Belly Rub N Grub 501(c)3 animal rescue needs fourteen days notice.

Adopter is fully aware that the agency makes no guarantees whatsoever as to the health, temperament, mental disposition and training of the Dog/Puppy. We can tell you what we know, with our knowledge and experience with the Dog/Puppy . Adopter understands that adopting a Dog/Puppy, the Dog/Puppy will need training, patience and time to adjust. Adopter is agreeing to help this Dog/Puppy fit into their family and give him/her time to adjust and acclimate. Returning the Dog/Puppy will cause harm to the Dog/Puppy ability to adjust again.

The Dog/Puppy must wear a breakaway collar and an information tag at all times. I tag must be purchased upon adoption and the microchip must be registered with www.foundanimals.org. Belly Rub N Grub 501(c)3 animal rescue must be listed as an additional contact on the microchip. If the microchip is from another company, it must be registered with that company also. (AVID, Home again etc)

If the Dog/Puppy for any reason is at a shelter and is not claimed within 3 days by his/her owner, the owner will pay Belly Rub N Grub 501(c)3 $500 to go to the shelter and claim the Dog/Puppy. This fee will be paid within 7 days. The Dog/Puppy will not be returned to owner, if Belly Rub N Grub 501(c)3 animal rescue claims it.

Adopter is aware that the adoption is not final until a home check has been completed and Belly Rub N Grub 501(c)3 animal rescue has approved the home. A home check will be within 3 weeks of adoption. Puppies under 4 months old may have a second home check. Ignoring Belly Rub N Grub 501(c)3 animal rescue calls and attempts to perform a home check will result in Belly Rub N Grub confiscating the Dog/Puppy. The adoption fee will not be returned.
